Scholar Information Form

Before a scholarship check will be issued, recipients must complete and submit a Scholar Information Form.

Students will receive notification from the Scholarship Administrator if they have been selected to receive a scholarship award. NCCF will then email the student with instructions on how to complete the Scholar Information Form in the NCCF Scholarships Portal.

This form is important because it provides NCCF with information required to process the scholarship payment to the student’s school.

The form also provides students the option to upload their acceptance letter and answer questions to share with the donors who established and support the scholarship opportunity:

  • Name of scholarship, hometown and high school
  • Official name of college/university you will attend in the fall
  • Expected graduation date
  • Are you a first-generation student?
  • What do you hope to accomplish by pursuing higher education? What are your specific interests?
  • Why did you choose your college or university, and what do you look forward to (or enjoy) most about attending this institution?
  • What experiences or challenges in your life have influenced you the most and/or motivate you to succeed?
  • How can you best express your gratitude as a scholarship award recipient - now and in the future?
  • Do you give NCCF permission to use this information in our marketing and communications materials?
