Scholar Information Form
Before a scholarship check will be issued, recipients must complete and submit a Scholar Information Form.
Students will receive notification from the Scholarship Administrator if they have been selected to receive a scholarship award. NCCF will then email the student with instructions on how to complete the Scholar Information Form in the NCCF Scholarships Portal.
This form is important because it provides NCCF with information required to process the scholarship payment to the student’s school.
The form also provides students the option to upload their acceptance letter and answer questions to share with the donors who established and support the scholarship opportunity:
- Official name of college/university you will attend
- Expected graduation date
- Your hometown and state
- Are you a first-generation student?
- In one paragraph, describe your career goals, educational interests or what you’re most looking forward to as you pursue higher education.
- In one paragraph, describe a life experience or person that influenced you or your goals.
- In two to three sentences, please share anything you would like the donor or sponsor of this scholarship to know.
Students will also have the option to give NCCF permission to use their information in marketing and communications materials, and to upload a photo for possible inclusion in NCCF’s Scholar Spotlights series.
